The Sangamon County Department of Public Health is seeking applicants to perform duties primarily as a Community Health Worker. 

Responsibilities
  • Engage in the community through outreach to recruit high-risk clients.
  • Assist clients in the community—through home visits, attending medical appointments, and accessing other community-based resources. Although this position is based at the office, CHWs are very mobile and spend a majority of their time actively working with clients, providers, primary care teams, and other agencies in the community.
  • Build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as, community education, informal counseling, social support and advocacy.
  • Continuously expand knowledge and understanding of community resources and clinic services and programs provided.
  • Complete client-centered treatment goals in collaboration with clients and review progress in meeting goals routinely and at identified check-in times.
  • Provide assistance with medical and behavioral health referrals to ensure client access and follow-through.
  • Complete referrals sent to other providers or social services and follow-up with clients on outcomes and next steps.
  • Attend and actively participate in all project related trainings and project meetings
Requirements
  • A valid Class D Illinois Driver’s License with the ability to use personal automobile with proof of current insurance
  • Must work occasional evenings and weekends as assigned
  • Applicants must be a resident of Sangamon County or be willing to move into the County within 6 months of employment. 
  • Desirable Qualifications:
  • Certified Community Health Worker
  • Secondary language skills
  • Any combination of 1-3 years health/social services experience and/or education
  • Knowledge of local population demographics, assets and needs
  • Knowledge of local health disparities and resources to address those disparities.
  • Ability to conduct presentations on health and wellness activities and self-management of health concerns.
  • Experience with specific patient populations/issues is highly desired (i.e. criminal justice system, homelessness, severe mental illness, domestic violence, substance use, etc.)
  • Experience and/or training in substance abuse education, referrals, and treatment
Skills

* Strong public speaking, communication, and writing skills

Benefits

Work Hours 8:00 a.m. – 4:30 p.m. Monday- Friday
Salary range:  $50,267.00 - $62,833.00 depending on qualifications
Time off:  2 weeks paid vacation | 12 paid sick leave days 3 paid personal days   | 13 paid holidays per yr
