Vote by Mail Applications

Sangamon County voters may apply to vote by mail for all county elections (also known as Permanent Vote By Mail) or to vote by mail for the next election only. Permanent Vote By Mail voters receive ballots for each subsequent election while voters applying for the next election will only receive a ballot for that election and must apply again if they wish to vote by mail for other elections.

When filling out a Vote by Mail application, you MUST include the following information; Name, Address, Date of Birth, City, and Zip Code and party affiliation if pertinent.

Fill out this application for Permanent Vote By Mail.

Application to Print out and Mail in for the upcoming election
Print out this application, then fill out and mail back to the Election Office for a ballot for the upcoming election.  Please note that applications will not be accepted after Thursday, March 27th.
