Massage Establishment License

Cost

• An application fee of $50.00 must be accompanied with the application (valid photo ID required when paying with a personal check or credit card). 
• An additional $55.00 background check and fingerprinting fee will need to be paid at the time of appointment at the Regional Office of Education. 
• Upon approval of the application, a $300 fee will be paid to the Sangamon County Clerk’s Office (valid photo ID required when paying with a personal check or credit card).
