The Sangamon County Department of Public Health is looking to hire an LPN/Case Manager-Infectious Disease
Responsibilities	Assist with immunization clinics including annual flu vaccination clinics at SCDPH. Make home visits to clients with reportable communicable diseases Attend seminars, workshops, and educational trainings related to infectious disease for professional growth and grant requirements Complete and report epidemiological investigations on reportable communicable diseases as required by Illinois Department of Public Health (IDPH) Control of Communicable Disease Code Interact with community stakeholders to provide guidelines and current recommendations about disease prevention. This includes health care providers, child care providers, school systems, private businesses, and the general public Investigate suspected and confirmed foodborne illness cases throughout Sangamon County. Requires consultation with the environmental health department staff regarding ill food service workers Monitor Class II and Class III TB clients for medication and medical follow-up compliance The work assigned to this position may not include all possible tasks in this description and does not limit the assignment of any additional tasks for this position.
Requirements	A valid Class D Illinois Driver’s License Applicants must be a resident of Sangamon County or be willing to move into the County within 6 months of employment Ability to use personal automobile with proof of current insurance Current State of Illinois License of Practical Nursing On rare occasions or in the event of a public health emergency, may be required to work evening and/or weekend clinic as assigned
Skills	Excellent communication skills Ability to utilize Microsoft Suite programs Ability to develop and maintain strong relationships with employees and other stakeholders
Benefits	Work hours: 8a-430p. Salary: $52,424.32. $26.88/hr 2 weeks paid vacation, 12 paid sick leave days, 3 paid personal days, 13 paid holidays per year Health, dental & life insurance. Mileage reimbursement, death benefits, retirement
