Before You Begin

Use a scanner, your smartphone, or a digital camera to capture images of each of the following requested documents.

• Birth Certificate
• Social Security Card (or proof that you have applied for one)
• Driver's License (if you have one)
• Firearm Owner ID Card (if you have one)
• DD214 (if you served in the military)
• College Transcript(s) (if you have a copy)

Alternatively, you can print out a paper copy of our application form, fill it out, attach a copy of each of the requested documents, and return it via:
