The Sangamon County Department of Public Health is seeking applicants to perform duties part-time as an Early Intervention Liaison and part-time as a Local Interagency Council (LIC) Coordinator which will create one full-time position. 

Responsibilities

Parent Liaison
o Helps families navigate the Early Intervention System
o Participate in Individualized Family Service Plan meetings to support families
o Provide information regarding insurance benefit use
o Facilitating parent support groups and communicate with provider team regarding family needs and appropriate referrals for counseling
o Coordinate opportunities for parent linkages and promote family centered policies and practices


Local Interagency Council Coordinator
o Maintain an up-to-date list of active members and routinely communicate related information to group members
o Ensure mandates for child find, public awareness, provider recruitment and transition are met
o Facilitate community needs assessment and providing administrative support for LIC meetings
o Design and implement Child Find and public awarness initiatives and activities
o Liaison between CFC offices, health care providers, and community agencies
o Facilitates Local Interagency Council Meetings, maintain detailed records
o Facilitates public awareness on EI Services and resources by attendance at community events, fairs, and private/public partners
o Tracks early childhood demographics and statistics, submit data reports
o Transition planning-ensure smooth transition of children into school district services and additional community support resources

Requirements

A valid Class D Illinois Driver’s License
 Applicants must be a resident of Sangamon County or be willing to move into the County within 6 months of employment.
 Ability to use personal automobile with proof of current insurance
 High school diploma
o OR General Education Diploma (GED)
 Eligible for Early Intervention Credential
 Eligible for Early Intervention Parent Liaison Credential.
 Parent of a child with a disability.
 Demonstrated experience in word processing software and/or database/spreadsheet software

Skills

* Strong organizational skills
* Experience in public health setting

Benefits

Work Hours are 8a-430p Monday -Friday
Salary range -$41,603.80-$52,004.75/year
2 weeks paid vacation, 12 paid sick leave days, 3 paid personal days, 13 paid holidays per year
