The Sangamon County Department of Public Health is seeking applicants to perform duties as a Community Navigator for the Illinois Breast and Cervical Cancer grant program (IBCCP). 

Responsibilities
  • Educating and building relationships with medical providers to expand available services to clients in a 10 county area;
  • Assessing and surveying medical providers to gather information regarding screening practices, tracking mechanisms, and other information that could support expansion of IBCCP services and completing needs assessments to determine gaps and barriers to care;
  • Conducting outreach activities to disseminate breast and cervical cancer related materials to targeted groups and visiting community worksites to encourage screening and/or recruiting women in targeted groups to obtain a screening;
  • Determining insurance status and needs of clients and navigating them through the health care system;
  • Assessing client barriers and assisting clients to reduce those barriers with scheduling assistance, transportation, dependent care, translation services, etc.;
  • Completing required reports and other documents to the Grantor;
  • Attending appropriate meetings, workshops, and seminars;

Requirements

* A Valid Class D Illinois Driver's License
* Applicants must be a resident of Sangamon County or be willing to move into the County within 6 months of employment. 
* Ability to use personal automobile with proof of current insurance.
* Able to travel at least 50% of their work schedule.
* Bachelor's degree preferred
* On rare occasions, this position requires evening and/or weekend work as assigned.

This position is grant funded and is subject to continued funding.

Skills

* Knowledge of public health and health systems including Medicaid/Medicare, HMOs and Federally Qualified Health Centers
* Strong organizational and communication skills
* Ability to collaborate and maintain relationships with relevant stakeholders

Benefits

Salary range $41,603.80 to $53,004.75; 2 weeks paid vacation, 3 paid personal days, 12 paid sick leave days and 13 paid holidays per year; 4 weeks Maternity/Paternity paid time off; Family Medical Leave Act (FMLA).  Individual and family health, dental and life insurance options, retirement, death benefits and mileage reimbursement.

Work hours: 8a-430p, Monday through Friday
